29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:  a) Warehouse: The Employer did not ensure the forklifts were properly maintained. The Linde Model #H20T serial 0054/2019 forklift,  the front left tire had multiple areas where the tire had pieces (chunks) missing. This condition was noted in the maintenance logs and the equipment was still in operation at the time of the inspection. Observed on or about 11/01/22.  CHEP Recycled Pallet Solutions, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29CFR 1910.178 (p)(1), which was contained in OSHA inspection number 1553199, citation number 1, item number 1 and was affirmed as a final order on 03/29/2022, with respect to a workplace located at 3011 Clark Ave. Albany, GA 31705.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Logs were provided late and had missing information that included hours worked and the proper amount of incidents that were required to be reported.  The subsequent request yielded logs that were not correct.
