29 CFR 1926.451(b)(5)(i):The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end: A) On or about 11/01/2022, Rehabilitation of Roosevelt Ave Bridge Over Van Wyck Expressway. Flushing, NY Employees working from a scaffold referred to as a temporary shielding approximately 13 feet above the Van Wyck Expressway were exposed to a fall hazard. A double channel steel support beam was displaced approximately 1 foot to the east on the underside of the shielding causing one end of three 3" x 10" x 52" long planks to become unsupported.

Flushing, NY Employees working from a scaffold referred to as a temporary shielding approximately 13 feet above the Van Wyck Expressway were exposed to a fall hazard. A double channel steel support beam was displaced approximately 1 foot to the east on the underside of the shielding. The underside of the shielding was not inspected by a competent person before each work shift.

Flushing, NY Employees working from a scaffold referred as a temporary shielding approximately 13 feet above the Van Wyck Expressway were exposed to a fall hazard. A double channel steel support beam was displaced approximately 1 foot to the east on the underside of the shields. The end of three planks were not supported by the beam and were not immediately repaired or replaced.
