29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of hazardous energy when employees were engaged in activities such as servicing and/ or maintenance of equipment.  In the production line, employees engaged in preventive maintenance activities on the Kataoka Machine - Duplex Splitter SL-KE70 and the Kataoka Machine - In-Line Winder IW-CD800 were exposed to crushing hazards and electrical shock hazards. Lockout /tagout procedures were not being utilized to control all energy sources of the machine prior to maintenance work being performed.

29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  In the production line, employees performing servicing and maintenance activities on Kataoka Slitter Rewinder SL-KA800 and Kataoka In-Line Winder IW-CD800 which had multiple energy sources including mechanical, pneumatic, hydraulic, electric, gravity, and water were exposed to crushing and electrical hazards. Employer had not conducted a periodic inspection of the energy control procedures at least annually.

Employee(s) operating Kataoka Machine - In-Line Winder IW-CD800 were exposed to caught-in and struck-by hazards in that the build-in guards on two rotating gears on the machine were left open.
