29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns. At the jobsite, employees were not wearing hardhats while working below other employees that were decking a residential house, exposing employees to struck-by hazards from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. At the jobsite, employees engaged in decking activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 16 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: At the jobsite, the employer did not provide a training program with hazard recognition and avoidance training where employees were performing decking activities above 6 feet without fall protection and exposed to fall hazards at a heights of approximately 16 feet to the ground below.
