29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system.  Location: 1050 Waltham Street, Lexington, MA  On or about 11/15/22, an employee was exposed to a fall hazard of 21-feet from a low-sloped roof to a lower level without being tied off to a fall protection system.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.  Location: 1050 Waltham Street, Lexington, MA  On or about 11/15/22, employees were allowed to begin work on a narrow low-slopped roof that was 21-feet above the ground below, which necessitated the use of fall protection which the employer had not install prior to the work being started.
