29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: At a temporary jobsite located at 9910 Widmer Rd., Lenexa, Kansas, employees engaged in millwrights activities with the use of a power industrial truck, Genie GTH-636, serial number GTH06E-13437, were exposed to struck by hazards. The forklift operator had not received refresher training and evaluations every three years.

29 CFR 1926.251(c)(9): Slings were not padded or protected from the sharp edges of their loads: At a temporary jobsite located at 9910 Widmer Rd., Lenexa, Kansas, employees performing millwright activities were exposed to struck by and crushing hazards in that a wire rope sling was used without padding or protection from the sharp edges of the deconstructed steel. The steel sling was strung through a "blown out" hole, exposing the sling to contact damage from the sharp edges of the hole.
