29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):   (a)Lloyd Companies, LLC dba Lloyd Construction Company at 7700 South Brett Avenue, Sioux Falls, SD 57108: On or about and at times prior to November 25, 2022, the controlling employer did not initiate or maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment made by a competent person. Subcontracted employees were conducting framing operations on a residential property and exposed to various hazards to include, lack of fall protection, improper ladder usage, uncertified forklift operations, and lack of training.  This condition exposed employees and subcontracted employees to falls, struck-by, and training hazards.

1926.503(a)(2)(i): Employees were not trained in the nature of fall hazards in the work area.   (a)Lloyd Companies, LLC dba Lloyd Construction Company at 7700 South Brett Avenue, Sioux Falls, SD 57108: On or about and at times prior to November 25, 2022, the controlling employer did not ensure that subcontracted employee were protected from falls from heights, in that subcontracted employees were conducting framing operations on a multi-family residential property with no protective system in place. This condition exposed employees to a fall hazard greater than six feet.
