29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the employer's energy control procedures required by this standard:      (b)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure specific energy control procedures to deenergize the machine were utilized which resulted in exposure to hazards related to pieces of equipment with multiple energy sources including, but not limited to the Jagenberg Diana 105 gluing machine. This condition exposed employees to crushing, amputation, shock, and air embolisms while maintaining equipment.    (c)Bell Incorporated at 609 West Algonquin Street, Sioux Falls, SD 57104: On or about and at times prior to November 30, 2022, the employer did not ensure specific energy control procedures to deenergize the machine were utilized which resulted in exposure to hazards related to pieces of equipment with multiple energy sources including, but not limited to the Jagenberg Diana 98 HS gluing machine. This condition exposed employees to crushing, amputation, shock, and air embolisms while maintaining equipment. This condition exposed employees to crushing, amputation, shock, and air embolisms. Employees regularly use a blow guns to clean off working surfaces of South Print Press area and general maintenance cleaning.   This condition exposes employees to embolisms, hearing damage, and eye and body injuries from flying debris.
