29 CFR 1926.1404(m)(1)(i): The selection of components, and configuration of the equipment, that affect the capacity or safe operation of the equipment was not in accordance with manufacturer instructions, prohibitions, limitations, and specifications: On or about December 6, 2022, a Terex CC2800-1, 660-Ton Crawler Mounted Lattice Boom Crane, was used at Phillips 66 Wood River Refinery to place a Kerosene Hydrotreater Reactor. The crane components and configuration were not in accordance with manufacturer specifications. Capacity and safe operation of the equipment was affected. a. The SuperLift counterweight frame was not connected to the SuperLift support rods with manufacturer specified rods. b. The SuperLift counterweight frame was not connected to the crane superstructure with manufacturer specified connecting ropes.
