The employee conducting masonry activities was utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards of approximately 45 feet.

The employee was required to utilize the scaffold that had not been inspected by a competent person, thereby exposing the employee to fall hazards.
