29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 23, 2022, and at times prior thereto, an employee was working in close proximity to rollers that were engaged on the conveyor system of a CRS 335 guillotine shear; exposing employees to the hazard of being caught-in the rollers.

On or about November 23, 2022, at this establishment, an employee suffered a workplace injury resulting in an inpatient hospitalization/amputation which was not reported to OSHA within 24 hours.
