29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  (a)  On or about November 16, 2022, employees were exposed to struck-by and fall hazards created by having no means of egress while working inside a trench in excess of five feet deep.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a)  On or about November 16, 2022, employees were exposed to cave-in hazards while working inside a trench that was 14 feet deep with near vertical walls in that, an adequate protective system was not in place where an insufficient number of shores were used and the east and west walls were not protected.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems:  (a)  On or about November 16, 2022, a competent person did not conduct an adequate inspection of the trench and protective system to evaluate the working conditions while employees were working inside a trench, 14 feet deep, that did not have an adequate protective system in place and had no means of ingress and egress.
