29 CFR  1904.7(b)(3): The employer did not correctly classify the following work-related injuries or illnesses on the OSHA Form 300 or equivalent.   At the Amazon EWR4 Fulfillment Center, Robbinsville, New Jersey, the employer did not correctly record the following workplace injuries or illness on the OSHA Form 300 or equivalent for the calendar year 2023:   a) Starting on or about 3/2/23, the employer failed to classify a work-related injury as an injury resulting in days away from work, and failed to record the number of days away from work. An employee working in the Pack Singles department was injured on 1/4/23 from repetitive lifting of heavy items. The employee had days away from work during the time period between surgery on 3/2/23 and return to work on 5/29/23.   The Amazon Fulfillment Services was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.7(b)(3), which was contained in OSHA inspection number 1589647, citation number 1, item number 1 and was affirmed as a final order on 10/3/2022, with respect to a workplace located at 550 Oak Ridge Road Hazleton, PA 18202.

29 CFR  1904.7(b)(4):The employer did not enter the correct number of calendar days of a work-related injury or illness on the OSHA Form 300 or equivalent resulting in restricted work or job transfer .    At the Amazon EWR4 Fulfillment Center, Robbinsville, New Jersey, the employer did not correctly record the following workplace injuries or illness on the OSHA Form 300 or equivalent for the calendar year 2022:   Starting on or about 3/1/23, the employer failed to accurately enter the correct number of days of restricted work for an employee who suffered a hand injury on 12/19/22 while working on packing in the AFE induct and rebin. The employee performed restricted work as a result of the injury during the time period March 1, 2023 to April 1, 2023 which was not included in the OSHA 300 totals.   Abatement Certification and Documentation is required
