29 CFR 1910.106(b)(6): In locations where, flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: a. At a temporary jobsite located at 735 E Washington Street, in Monticello Florida: On or about December 22, 2023, and times prior, the employer exposed employees to fire and explosion hazards, in that the employer failed to control sources of ignition such as, but not limited to, static electricity while employees were engaged in cleaning storage tanks containing flammable liquids.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a. At the facility, located at 735 E Washington Street, in Monticello Florida: On or about December 22, 2023, and at time prior, the employer exposed employees to injuries and illnesses caused by hazardous chemicals, in that the employer did not provided information and training on hazardous chemicals for employees while handling, cleaning, and transporting flammable and combustibles liquids chemicals such as, but not limited to, kerosene, diesel, and gasoline.
