29 CFR 1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.   On or about January 5, 2023, and times prior to, the floor accumulated oil and water, exposing employees to trip and slip hazards in the walkways in the lower bay area.

On January 5, 2023, and at times prior to, the employer did not inspect and provide maintenance on the following portable fire extinguishers in the workplace:  a) The fire extinguisher located on the east side of the upper bay; b) The fire extinguisher located in the middle of the upper bay next to the eye wash station.
