1910.145(c)(1)(ii)  All employees shall be instructed that danger signs indicate immediate danger and that special precautions are necessary. 29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Processing Department: The employer did not ensure procedures were utilized to de-energize the Rotar Claw while maintenance was performed. The unguarded chain and sprocket was running while being greased, exposing the operator to an amputation hazard.
