29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a. Richmond Machine Company - Montpelier, OH: On or about December 28, 2022, employees that operated the Baileigh Industrial roll bender machine (model PR 413 s/n 46149) were exposed to amputation injury.  The employer did not ensure the machine's point of operation was adequately guarded.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR  1910.243(c)(3):Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:  a. Richmond Machine Company - Montpelier, OH: On or about January 3, 2023, the employer did not assure that the Metabo 4 ?" right angle hand grinder used in the northeast fabrication bay to grind welds on an aluminum water tank was equipped with a guard to protect employees from the rotating abrasive wheel.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
