29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a.) On or about December 12, 2022 , at 2155 E. Cherokee Street, Bartow Florida 33830, employees working on walk boards placed between three A frame ladders ("L shape") were not adequately trained to recognize hazards associated with this type of scaffolding used.

Cherokee Street, Bartow Florida 33830, employees were exposed to fall hazards, in that, they used walk boards set up on three A-frame ladders ("L shape"), in that, the ladders were used for purposes other than what they were designed for.

29 CFR  1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a.) On or about December 12, 2022 , at 2155 E. Cherokee Street, Bartow Florida 33830, the employer failed to report a work-related in-patient hospitalization of an employee to OSHA within twenty-four (24) hours. The employer notified OSHA on January 13, 2023, of the in-patient hospitalization.
