On or about 01/20/2023, at 6000 W Glade Rd, Unit 1051A, Boca Raton, Florida, employer did not remove the water stained cracked and discolored ceiling tiles due to water leaking from the roof exposing employees to slip, trip and fall and struck by hazards.
