29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c):    a) Job site, 143 Market Place Drive, Coraopolis, PA: On or about January 24, 2023, during sanitary line installation activities in an excavation determined to be at least approximately 9 feet deep, the employer did not ensure that appropriate cave in protection was in use along the upper portions of the excavation in that the excavation walls were determined to be vertical/near vertical and soil types were determined to be no better than a class B soil, exposing employees to cave in hazards.

29 CFR  1926.651(k)(2):  Where the competent person found evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions had been taken to ensure their safety:    a) Job site, 143 Market Place Drive, Coraopolis, PA: On or about January 24, 2023, during sanitary line installation activities in an excavation determined to be at least approximately 12 feet deep, the employer did not ensure that the competent person removed an employee from an excavation where safety issues existed. Deficiencies noted during the inspection included, but were not limited to, allowing the employee to finish cutting stone inside the trench box where the excavation walls were collapsing, where too much room between the trench box and the excavation walls existed and where the means of access/egress was located outside of the trench box.
