Deficiencies noted during the inspection included, but were not limited to, allowing the employee to finish cutting stone inside the trench box where the excavation walls were collapsing, where too much room between the trench box and the excavation walls existed and where the means of access/egress was located outside of the trench box.

Soils throughout the excavation overall were determined to be no better than a class "B" soil, exposing employees to hazards including, but not limited to, cave in hazards.
