Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked.   A) 4000 Bordentown Ave STE 2 Sayreville, NJ 08872 - The employer did not ensure the segregation of pedestrians from forklift traffic at the loading docks.  Incident occurred on January 24, 2023

A) 4000 Bordentown Ave Suite 2 Sayreville, NJ 08872 - Loading Dock:   The forklift operator did not maintain a clear view of the path of travel and struck an employee who was in the loading dock area.   Incident occurred on January 24, 2023.
