29 CFR  1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  (a) Lot 196/197 Buttonbush Lane, Gettysburg PA, front of the house - On or about January 27, 2023, Employees were performing work on a residential house with generator power to electrical tools, where an energized extension cord with an damaged outer jacket was being used to energize a Dewalt circular saw.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR  1926.502(d)(17):  The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:    (a)  Lot 196/197 Buttonbush Lane, Gettysburg PA, on the front roof - On or about January 27, 2023, employees were performing work on a residential rooftop with a height of between approximately 10 feet up to 11 feet above grade at the lower edge of the roof, without proper fall protection. Employees had fall protection however the D ring was not located between the employees shoulder blades, and was located in the small of their back, employees were exposed to fall hazards.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  (a)  Lot 196/197 Buttonbush Lane, Gettysburg PA, on the front roof - On or about January 27, 2023, employees were performing work on a residential rooftop with a height of between approximately 10 feet up to 11 feet above grade at the lower edge of the roof, while utilizing fall protection equipment rigged to allow a free fall greater than two feet.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
