Location: Facility Garage Area On or about January 30, 2023, an employee was struck by and caught between a flatbed truck and a machine attachment that was being hoisted by a CAT TH210 Telehandler while working and was fatally injured.

Location: Facility Yard and Garage Area On or about January 30, 2023, an employee and the CAT 210TH Telehandler operator were exposed to struck-by and overturning hazards while hoisting and transporting an unstable asymmetrical load with a single leg chain sling. The employee guiding the telehandler was struck-by the load being hoisted and pinned between a flatbed truck and in result was fatally injured.
