29 CFR 1910.147(a)(3)(i): The employer did not establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees:  (a)	This violation occurred on or about February 10, 2023, and times prior thereto, where the employer did not ensure an energy control program were developed and implemented.

29 CFR 1910.212(a)(1): One or more machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying, chips and sparks:  (a)	This violation occurred on or about November 7, 2022, where the employer did not ensure the Dodd Dovetail tooling, model no. SE-1CNC-D, serial no. D15043-120, was adequately guarded. Employees were exposed to an amputation hazard.
