Roof area of the residential job site located at 5741 Merrill Road, Florida 32277: On or about February 21, 2023, the employer exposed employees to eye injuries caused by air-born debris and flying objects, in that eye protection was not utilized by the employees, while tearing off old roof shingles and while installing sheathing using pneumatic nail guns.

At a residential job site located at 5741 Merrill Road, Jacksonville, Florida: On or about February 21, 2023, the employer exposed five employees to fall hazards ranging up 15-foot, in that fall protection was not utilized by employees while installing roof shingles.
