Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about February 23, 2023, the employer did not ensure employees were protected from a fall of over 6 feet by the use of guardrail systems, safety net systems, or personal fall arrest systems, while performing roofing work at a non-residential location located at 8206 Euclid Avenue, Cleveland, Ohio 44113.
