29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On February 22, 2023, employees used extension ladders that did not have the side rails extended three (3) feet over the upper landing surface and were exposed to potential fall hazards greater than six feet.

29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.  On February 22, 2023, employees using an extension cord and power cord that had the outer sheathings pulled away exposing internal wiring.  Employees were exposed to electrical shock hazard.  a)  Extension cord used to energize the air compressor outer sheathing was exposed, b)  The air compressor power cord outer sheathing was not connected to the terminals.
