29 CFR  1910.140(d)(2)(ii):29 CFR 1910.49(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level:  A) Berner International LLC., on or about February 23, 2023 in the storage area of the building: Employees operating a Crown SP picker were provided and required to wear fall protection system consisting of a six foot lanyard and body harness. The fall protection system was not designed and rigged to prevent contact with the concrete floor at falls less than 10 feet above ground level.
