29 CFR  1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about February 24, 2023, suitable facilities for quick drenching or flushing of the eyes and/or body were not provided exposing employees to eye hazards during workover operations.

29 CFR  1910.157(e)(3):The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.  The annual maintenance date was not recorded and retained as required for fire extinguishers on Rig No. 52.

29 CFR  1910.1200(f)(6)(ii): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with product identifier and words, pictures, symbols, or combination thereof, which provided at least general information regarding the hazards of the chemicals:  Employees used black bottle containing diesel antifreeze that  lacked labeling for its contents and hazards, exposing employees to poisoning hazards.
