On or about February 16, 2023, at an establishment located at 1015 Centennial Street in Green Bay, Wisconsin, in Bay #1 by the Cincinnati 750H hydraulic brake press; employees were exposed to struck-by and crushing hazards while utilizing an unapproved indentation-type screw clamp to lift a steel plate that weighed approximately 550 lbs. into the brake press for bending. The steel plate slipped out of the clamp as it was being lifted out of the hydraulic brake press, dropping the part onto an employee's leg.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   At an establishment located at 1015 Centennial Street in Green Bay, Wisconsin:  a) On or about March 22, 2023, to March 23, 2023, in the machining area; operator employees were exposed to amputation and struck-by hazards created by the blade and the rotating chuck and parts being cut by the King Vertical Turret Lathe (VTL) while cutting sheet metal parts. The machine was not effectively guarded to prevent exposure to these hazards.  b) On or about February 16, 2023, to March 29, 2023, employees were exposed to amputation and struck-by hazards created by the unguarded clamps on the Miller Welding Automation seamer (Model: ALC-401-60D; S/N: 2021-153).  c) On or about March 29, 2023, employees were exposed to struck-by hazards created by the movement of the operator's station on the ALLtra plasma cutting table when it returned to its home position.

The machine was not effectively guarded to prevent exposure to these hazards.
