29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition: a) Electrical Assembly Area, 154 Eastbrook Lane, Butler, PA 16002: On or about February 13, 2023, the employer did not ensure that the Electrical Assembly area was kept in a clean and orderly condition. An air hose stretched across the floor between a Global brand hose reel and an Advance Lifts brand lift table created a trip hazard to employees working in the area.

29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: a) Jobsite, 154 Eastbrook Lane, Butler, PA 16002: On February 13, 2023, an employee tripped over an air hose and fell resulting in serious injuries. The employee passed on February 17, 2023 from the injuries caused by the fall. The employer did not report the death of the employee within the required 8 hours.
