The employer has not established a program consisting of energy control procedures, employee training and periodic inspections. b) Production area: On or about February 28, 2023, and at times prior, the employer exposed an employee to struck-by and caught-in between hazards, in that the employee is required to change a die set on a Beckwood hydraulic press machine without following specific lock-out procedures. The employer has not established a program consisting of energy control procedures, employee training and periodic inspections.
