29 CFR  1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  (a)Nutrient Farms, 364 Storm King Rd, New Castle, CO 81647: On or about February 27, 2023, and at times prior, employees were exposed to amputation hazards when operating a handheld grinder without proper guarding. An employee was cutting metal rods using a saw that had the guard removed.

29 CFR  1926.200(h)(1): 29 CFR 1926.200(h)(1):  Accident prevention tags were not used a temporary means of warning employees of an existing hazard, such as defective tools, equipment, etc.   (a) Nutrient Farm, 364 Storm King Rd, New Castle, CO 81647: On or around, at at times prior, employees were exposed to crushed by hazards when the employer allowed operators to drive a tagged out mini-excavator (CE004795) to complete digging for an irrigation pipe. Excavator had been tagged out for faulty hydraulics but employees continued to operate the machine.
