29 CFR 1918.35: Employees were working around open weather deck hatches which were not protected by coamings to a height of 24 inches (0.61 m), or by taut lines or barricades at a height of 36 to 42 inches (0.91 m to 1.07 m) above the deck. M/V NYK Orion, Berths 25-33, Port of Oakland: At Bay 14, forward, starboard, standard railings or other adequate fall protection was not provided on the catwalk adjacent to an open hatch as required. Employees had been working in this area to lash containers and observe container loading and were exposed to fall hazards in excess of 9" to the main desk and approximately 60' to ground. Employees are exposed to fall hazards.

29 CFR 1918.22(b): Each side of the gangway, and the turntable if used, did not have a handrail with a minimum height of 33 inches (measured perpendicularly from rail to walking surface at the stanchion) and a midrail. M/V NYK Orion, Berths 25-33, Port of Oakland: Approximately four rung lengths of gangway railing were missing top and midrails on the right-side ascending and slack on the left side entry point. Employees are exposed to slip, trip and fall hazards.

1918.22(c) The gangway shall be kept properly trimmed. 29 CFR 1918.22(c): The gangway was not kept properly trimmed M/V NYK Orion, Berths 25-33, Port of Oakland: The space between the foot of the gangway and the apron had a measured vertical gap in excess of one foot and was not substantially parallel to the dock. Employees are exposed to slip, trip and fall hazards.
