29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person:  (a)NuAge Builders, LLC at 100 Quality Park Drive, Beresford, SD 57004: On or about and at times prior to March 15, 2023, the employer did not ensure an accident prevention program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Subcontracted employees were conducting roofing operations on a commercial property without the use of a fall protection system, lack of fall protection while utilizing aerial lifts, forklift hazards, and lack of training. This condition exposed employees to fall hazards of greater than six feet.
