29 CFR  1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures.   Location - Production Area/ Charging Point on mezzanine: On or about 03/22/2023, employees were not trained on the new procedure to verify lack of pressure and remove cap safely for feed tube system.

29 CFR  1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation:  Location - Production Area/ Charging Point on mezzanine: On or about 03/22/2023, an employee was injured when stored pressure was not relieved before removing cap on feed tube system.
