29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  Location - 13 Nagog Hill Road, Acton, MA.  On or about March 23, 2023, an employee was permitted to use a Milwaukee M18 FUEL COMPACT BAND SAW against the manufacturer's instructions by holding a section of copper pipe with the opposing hand, near the blade cutting area, resulting in a severe injury to their thumb.
