(a) Logging Site: On or about March 20, 2023, and times prior, training on general safety and health hazards of logging operations is not being provided to the onsite employee. The employee has been working on the logging site for approximately three months and has not been trained on the hazards of logging or provided with personal protective equipment, such as, but not limited to head protection to protect the employee from overhead hazards, face and eye protection to protect the employee from flying objects, and leg protection to protect the employee from lacerations if accidental contact is made with the chainsaw cutting blades.

The in-patient hospitalization was not reported to OSHA by the employer within 24-hours.
