On or about April 5, 2023, employees were exposed to anhydrous ammonia release hazards when the employer did not ensure dual relief valves on Compressor #108 in the Cook Refrigeration Plant were replaced and/or recertified according to their employed RAGAGEP including but not limited to IIAR Bulletin 110 (Start Up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems)Section 6.6.3 Revision, May 24, 2007.
