29 CFR  1910.119(d)(3)(i)(D): Employer did not complete a compilation of written process safety information on the relief system design and design basis.   On or about April 5, 2023, employees were exposed to anhydrous ammonia release hazards when the employer did not ensure equivalent lengths on discharge termination piping for any and all relief valve discharged to atmosphere were verified according to their employed RAGAGEP; including but not limited to IIAR 2 1984 Section 3.11.4.13.

29 CFR  1910.119(j)(4)(iii):The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience.  On or about April 5, 2023, employees were exposed to anhydrous ammonia release hazards when the employer did not ensure dual relief valves on Compressor #108 in the Cook Refrigeration Plant were replaced and/or recertified according to their employed RAGAGEP including but not limited to IIAR Bulletin 110  (Start Up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems)Section 6.6.3 Revision, May 24, 2007.

29 CFR  1910.22(a)(3): (3) Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.   On or about April 5, 2023, employees were exposed to slip/trip hazards when the employer did not ensure the Cook Engine Room floor was free of protruding section of abandoned in place  piping.

29 CFR  1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces.  NOTE: A sign reading DANGER -- PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.   On or about April 5, 2023, employees were exposed to anhydrous ammonia hazards when the employer did not  inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces in the evaporative condensers in Cook and Main (Fresh) refrigeration plants.
