29 CFR 1910.30(b)(1): (1) The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment. a) On or about 04/11/2023, at the job site located at 439 W. Maple Ave Winter Garden, FL 34787, employees making preparations to cut trees were exposed to struck by and crushing hazards when an Altec Telescopic Articulating Aerial lift (model AT37GW serial #90-82060833-05-22) was being operated without the tracks being extended per the manufacturer's instructions.
