29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup, or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about 4/13/2023, Pratt Sawmill, LLC., had not developed a program consisting of control procedures, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup, or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, were not guarded in accordance with the standards specified in paragraphs (m) and (o) of this section.   On or about 4/13/2023, Pratt Sawmill, LLC., had not ensured pulleys and belts were properly guarded, exposing employees to caught in hazards while working in the proximity of the blower motor drive mechanism. 29 CFR 1910.303(b)(1):  Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.  On or about 3/29/2023, employees engaged in sawmill activities at the Pratt Sawmill, LLC., located at 2559 State Hwy E, Fredericktown, Missouri were exposed to electrical hazards due to the following:  a.  Control box missing buttons exposing employees to electrical wires. b.  Air compressor electrical box missing a cover exposing employees to electrical wires. c.  A dual outlet electrical junction box had openings on the top and side exposing employees to electrical wires.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions, and openings through which conductors enter were not effectively closed.   On or about 4/13/2023, employees engaged in sawmill activities at the Pratt Sawmill LLC., located at 2559 State Hwy E, Fredericktown, Missouri, were exposed to an electrical junction box and an equipment control box with openings that were approximately one inch in diameter.  The junction boxes were not effectively closed.
