29 CFR  1926.502(i)(3): All covers shall be secured when installed so as to prevent accidental displacement by the wind, equipment, or employees.   Employees were engaged in roofing activities at a new construction commercial building located at 1120 Ave E, Wisner, NE  68791, and were exposed to fall hazards in that the employees were working in excess of 6-feet while conducting roofing operations with unsecured hole covers that were protecting employees from falling into holes that are in excess of 37 and ?-inches wide.

29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  Guarantee Roofing & Sheet Metal Inc. failed to report an inpatient hospitalization of an employee following a fall incident at a new construction commercial building located at 1120 Ave E, Wisner, NE  68791 until after 10-days had passed since the incident occurred.
