29 CFR  1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   (a) On or about 4/18/2023 an employee was exposed to fall hazards and crushing hazards while using a RAVYMOND Model 5300 Order picker Lift Truck that had a make-shift desk that was fabricated in-house onto the man-lift platform made for lifting employees. Beverage Distributors Company, LLC did not have the manufacturer approval for the man-lift modifications/additions in writing.
