The employee had general safety training and several of the onsite supervision had specific fall protection training however the PFAS anchorage points were not set per manufacturer guidelines, lifeline slack calculations were not made per manufacturer guidelines, and the layout of the site overall made effective implementation of the chosen fall protection system ineffective.
