29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about 04/17/202, in Buildings 1 and 2 of a work site located at 1001 Baker Road in Stuart, Florida, an employee was exposed to fall hazards of approximately 20 to 30 feet when working near the openings of elevator shafts that did not have the elevator doors and/or guardrails placed in those openings to prevent employees from falling into shafts.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On or about 04/17/2023, in Buildings 1 and 2 of a work site located at 1001 Baker Road in Stuart, Florida, an employee was exposed to fall hazards approximately 20 to 30 feet into open elevator shafts prior to the employer conducting an inspection of the worksite to ensure that the elevator doors and/or guardrails were installed to prevent employees from falling into those unguarded shaft openings.
