29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.   On or about April 15, 2023, an employee was hospitalized after being injured while working at Costco Wholesale Corporation dba. Costco Wholesale. The employer failed to report the  in-patient hospitalization to OSHA within 24 hours of learning of the hospitalization. The employer learned of the hospitalization on April 15, 2023.  The employee passed away on April 17, 2023. OSHA learned of the fatality through the media on the evening of 19 April 2023. At the time of opening on 20 April 2023, the employer had not reported the in-patient hospitalization to OSHA.
