29 CFR  1926.1203(a): Before beginning work at a worksite, the employer did not ensure that a competent person identified all confined spaces in which one or more of the employees it directed may have worked, and/or did not identify each space that was a permit space, through consideration and evaluation of the elements of that space, including testing as necessary:   (a) 401 Bell Avenue, Mt Gretna, PA - On or about May 5, 2023, the employer did not properly evaluate the workplace to determine if the interior of the water tower under construction was a permit-required confined space or could potentially become a PRCS once welding fumes were introduced by welding activities on the interior surface of the water tower. The employer did not conduct air monitoring or provide forced air during welding operations but instead relied on the "natural ventilation" created by the two openings at the bottom of the tank and the opened top of the tank.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
