Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm, in that employees were exposed to amputation hazards caused by the improper use the Biro  Meat Saw:  a) ShopRite 1600 W. Newport Pike Wilmington, DE 19804, Meat Department: On or about April 14, 2023, an employee utilized a Biro  meat Saw, model-3334, serial 56757 to cut pigs feet resulting in an amputation. The Biro meat saw was not approved for use to cut pigs feet.

You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  a) Shoprite of First State Plaza 1600 West Newport Pike, Wilmington, DE 19804: On or about May 10, 2023, the employer failed during calendar year 2022, to electronically submit information from their OSHA Form 300A or equivalent by March 2, 2023. The establishment employed 1022 employees during calendar year 2022.
