29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  a) Paper Machine #1 first lower felt bottom; On or about May 11, 2023, and at times prior thereto the employer exposed employees to caught-in and amputation hazards in that employees were allowed to conduct maintenance and cleaning activities on the paper press without all employees applying a personal lock on the group lockout device.
