29 CFR  1926.602(d):Training did not consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace as required by paragraph 29 CFR 1910.178(l) AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d).  (a) 100 Cawthon Way, Lavonia, GA 30553; On or about May 11, 2023, near building 4, an employee operating the JCB forklift was not trained to operate it safely, exposing employees to a struck-by hazard.
